State-Specific Rules for Selecting Medical Providers:
Managed Care Arrangements - Employee selects initial/primary treating physician from among primary providers in the managed care provider network as contracted for by the Employer/Carrier. Employee may request, in writing, to change treating physicians one time during course of treatment for any one accident. F.S. 440.134 (2)(a), (6)(c) and (10)(c).

Non-Managed Care Arrangement - Employer/carrier provides initial medical treatment. Employer/Carrier directs/approves subsequent treatment if necessary. Employee may request, in writing, to change treating physicians one time during course of treatment for any one accident. Employee must receive approval from Employer/carrier for all medical treatment and before changing physicians. F.S. 440.13(2)(a)-(2)(f).
